Home Care Network, Inc. (HCN) seeks an experienced RN to join as a RN Case Manager (RNCM) / Recertification Nurse serving southeastern Ohio with travel to Jackson, Gallia, Vinton, Ross, and Pike counties.

Qualifications

• 1+ year in case management, admissions/discharges, wound care, PICC/PORT/IV care, and OASIS/non-OASIS assessments.
• 1+ year at home health agency or in-home care; 2–5 years preferred.
• Experience leading field staff and strong documentation.

Responsibilities

• Case management, SOCs, admissions, recertifications, ROC and clinical reviews.
• Mentor and train staff; contribute to training and education.
• Travel to clients across SE Ohio within a 1-hour radius; mileage reimbursement.
