Home Care Network, Inc. seeks a highly skilled RN Case Manager/Recertification Nurse to coordinate care for NW Ohio clients in a Part-Time or PRN capacity. You will travel regionally to manage cases, perform assessments, and ensure high-quality patient outcomes.

Qualifications

• Graduate of an accredited School of Nursing.
• Minimum 1 year experience as an RN in home health or equivalent setting.
• 1+ year in case management, admissions/discharges, wound care, and OASIS/ NON-OASIS assessments.

Responsibilities

• Coordinate and supervise patient care across NW Ohio counties.
• Travel to client visits with no overnight shifts.
• Maintain thorough documentation and care plans.
• Mentor and lead field staff; support training and quality initiatives.
