Description
American Traveler is hiring an experienced RN Case Manager for an acute care hospital, requiring 2 years of acute care case management/discharge planning experience and a CA RN license.
Details
- Acute care hospital setting within the Case Management department
- Day shift schedule, 5x8-hour shifts from 8:00 AM to 4:30 PM
- Patient ratios of 1:25 on weekdays and 1:45 on weekends
- Every other weekend work required to meet department needs
- No floating expected
- Travelers may float between sister facilities near the contracted site
Requirements
- Active CA RN license required; pending license accepted if licensed by start date
- Minimum 2 years of acute care case management/discharge planning experience required
- Must be a seasoned traveler with prior travel assignment experience
- Epic EMR experience required
- At least 2 supervisory references covering 1 year of employment within the last 3 years
- Full SSN and DOB required for consideration
Additional Information
- Responsibilities include admission criteria review, care coordination, discharge planning, disease management, needs assessment/DME ordering, plan of care development, and prior authorizations
- Familiarity with regulatory standards such as CMS, Department of Health, DRG, and The Joint Commission/Core Measures/National Patient Safety Goals expected
- Experience across specialties such as Emergency Department, ICU, Med-Surg, and Surgical Services relevant to caseload
- Business attire or scrubs acceptable dress code
- Must commit to working 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
- RTO requests limited to no more than 7 days and must be submitted upfront
- First-time travelers will not be considered for this role
- Former permanent employees may return as travelers after a 3-month waiting period, provided they reside outside a 50-mile radius of the facility
- Local candidates accepted at a lower bill rate
- Candidates must reside outside a 50-mile radius unless applying under the local rate policy
