Qualifications

• Active PTA license in Massachusetts.
• Graduate of an accredited PTA program.
• Prior SNF or geriatric experience preferred.
• Strong communication and documentation skills.
• Ability to work under supervision of a licensed PT.

Responsibilities

• Provide skilled physical therapy services under supervision of a licensed PT.
• Implement treatment plans to improve mobility, strength and independence.
• Deliver direct therapy sessions including exercises, gait training and balance activities.
• Observe and report resident responses and progress to the supervising PT.
• Assist with resident assessments and contribute to documentation and care planning.
• Educate residents and caregivers on exercises to promote safety and carryover.
• Maintain accurate documentation in the EMR and proper use of equipment.
• Participate in interdisciplinary team meetings and coordinate resident care.
• Adhere to state, federal, and facility regulations and professional standards.
• Uphold licensure requirements and pursue continuing education.

Responsibilities

• Provide skilled physical therapy services under the supervision of a licensed Physical Therapist
• Implement treatment plans to improve residents’ mobility, strength, balance, and functional independence
• Deliver direct therapy sessions, including therapeutic exercises, gait training, and balance activities, safely and effectively
• Observe and report resident responses to interventions, including progress or changes in condition, to the supervising PT
• Assist with resident assessments and contribute to documentation and care planning as appropriate
• Educate residents and caregivers on exercises and techniques to promote safety and carryover outside therapy sessions
• Maintain accurate, timely documentation in the electronic medical record and ensure proper use of therapy equipment and assistive devices
• Participate in interdisciplinary team meetings and collaborate with staff to support coordinated resident care
• Adhere to state, federal, and facility regulations and professional standards of practice
• Uphold licensure requirements and pursue continuing education as applicable

Requirements

• Active Physical Therapist Assistant license in Massachusetts
• Graduate of an accredited PTA program
• Prior experience in SNF or geriatric settings preferred
• Strong communication, clinical judgment, and documentation skills
• Ability to work effectively as part of a rehabilitation team under supervision of a licensed PT
• Ability to work in a long-term care / skilled nursing facility environment
