The position features 8-hour day shifts (8:00 AM - 4:30 PM), 1:1 patient care, and possible weekend coverage. Travel within a 60-mile radius is common; mileage is reimbursed. Home Health software (HCHB) and Rover app usage are required; business casual attire is standard.

Qualifications

Responsibilities

• 8-hour day shift (8:00–16:30), 5 days/week; weekends may be required.
• Use HCHB, Remote Client, and Rover app for visits.
• Group visits when possible; travel up to 60 miles or 60–90 minutes daily.
• Mileage reimbursed at IRS standard rate; business casual attire.

• Minimum 1 year of skilled Home Health experience required
• BLS certification required
• HCHB (Homecare Homebase) proficiency required
• Reliable personal transportation and valid driver's license required

Responsibilities

• Skilled Home Health setting with 1:1 patient ratio
• Approximately 30 productivity points per week, up to 6 patients per day
• 8-hour day shift schedule (8:00 AM - 4:30 PM), 5 days per week
• Weekend support may be required
• Uses Home Care Home Base (HCHB), Remote Client for Home Health on laptop, and Rover phone application
• Facility makes efforts to group patient visits and minimize driving, though service area may extend up to 60 miles or 60-90 minutes of total daily driving
• Mileage reimbursed at current IRS standard rate
• Business casual attire required (no jeans); scrubs permitted but optional with no specific color requirement
• Free parking available
• First-time travelers accepted
• Up to 3 days of RTO may be automatically approved, with blackout periods during the weeks of Thanksgiving, Christmas, and New Year's
• Local clinicians accepted within a 50-75 mile radius at the same rate
• Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
• Travel pairs accepted, with each traveler assigned to their own territory and required to have their own transportation
