Job description

American Traveler is hiring an LPN for a skilled home health position requiring at least 1 year of home health experience and HCHB proficiency.

Requirements

• Active Oregon LPN license required before start date; license required if applicable; temporary licenses not accepted
• Minimum of 1 year of skilled home health experience required
• HCHB (Homecare Homebase) proficiency required
• OASIS experience required for staff completing Start of Care (SOC)
• BLS certification required
• Reliable personal transportation and active driver's license required

Responsibilities

• Skilled home health setting providing 1:1 patient care
• Approximately 30 productivity points per week, seeing up to 6 patients per day
• 8-hour day shift schedule (8:00 AM-4:30 PM), 5 days per week
• 4x10 and 3x12 schedules are not available
• Weekend support may be required
• Service area may extend up to approximately 60 miles or 60-90 minutes of total driving per day
• Documentation completed using Home Care Home Base (HCHB), a remote client laptop application, and the Rover phone app
• First-time travelers will be considered for this role
• On-call rotation may be required; details to be discussed during interview
• Facility groups patient visits to minimize driving time
• Mileage reimbursed at the current IRS standard rate
• Business casual attire required; no jeans; scrubs permitted but not mandatory
• Up to 3 days of RTO may be automatically approved, though blackout periods apply during the weeks of Thanksgiving, Christmas, and New Year's
• Local candidates accepted within a 50-75 mile radius at the same pay rate
• Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
• Travel pairs accepted, though each traveler is assigned their own territory and must provide their own transportation
• Completion of a Hospice/Home Health checklist is mandatory
• Monday orientation is required
