Qualifications

• Must have reliable transportation and a valid driver's license.

Responsibilities

• Skilled Home Health setting with 1:1 patient ratio.
• Up to 6 patient visits per day with ~30 productivity points per week.
• Day shift schedule, 5x8 hours (8:00 AM-4:30 PM).
• Weekend support may be required; no on-call requirement.
• Uses Home Care Home Base (HCHB), Remote Client for Home Health on laptop, and Rover phone application.
• First-time travelers accepted; local clinicians within 50-75 miles at the same rate.
• Service area may extend up to ~60 miles or 60-90 minutes of driving per day.
• Facility works to group patient visits to minimize driving time.
• Up to 3 days of RTO may be automatically approved.
• Blackout periods during Thanksgiving, Christmas, and New Year's.

• Minimum 1 year of skilled Home Health experience required
• HCHB (Homecare Homebase) proficiency required
• BLS certification required
• Must have reliable personal transportation and a valid driver's license

Responsibilities

• Skilled Home Health setting with 1:1 patient ratio
• Up to 6 patient visits per day with approximately 30 productivity points per week
• Day shift schedule, 5x8 hours (8:00 AM-4:30 PM)
• 4x10 and 3x12 schedules are not available
• Weekend support may be required
• No on-call requirement
• Uses Home Care Home Base (HCHB), Remote Client for Home Health on laptop, and Rover phone application
• First-time travelers accepted
• Local clinicians accepted within a 50-75 mile radius at the same rate
• Service area may extend up to approximately 60 miles or 60-90 minutes of total driving per day
