Job summary

American Traveler is seeking a Home Health RN in Oregon’s mid-Willamette Valley region. This role requires an active OR RN license, BLS, and at least 1 year of skilled home health experience.

You will work a day shift with 1:1 patient care, travel within a limited radius, and use HCHB, OASIS, and Rover for documentation and communication. Expect mileage reimbursement, on-site parking, and potential weekend coverage.

Responsibilities

• Provide skilled Home Health care with 1:1 patient ratio
• Manage up to 6 patients per day
• Reconcile approximately 30 productivity points weekly
• Work day shift 8:00 AM–4:30 PM
• No 4x10 or 3x12 shifts; weekends may be required
• Travel up to 60 miles/day with own transportation
• Utilize HCHB, Remote Client for Home Health, Rover, and OASIS
• Group visits to minimize driving
• Free on-site parking; mileage reimbursed per IRS rate
• First-time travelers accepted for this position

Job description

American Traveler is hiring a Home Health RN for a skilled home health position requiring an active OR RN license and at least 1 year of skilled home health experience.

Requirements

• Active Oregon RN license required before start date; temporary licenses not accepted
• Current BLS certification required
• Minimum 1 year of skilled Home Health experience required
• OASIS experience required for completing Start of Care (SOC) visits
• Proficiency in Home Care Home Base (HCHB) required
• Must have a valid driver's license and reliable personal transportation

Responsibilities

• Skilled Home Health setting serving a mid-Willamette Valley service area
• 1:1 patient ratio with up to 6 patients per day
• Approximately 30 productivity points per week
• Day shift schedule, 8-hour shifts (8:00 AM-4:30 PM)
• 4x10 and 3x12 schedules are not available
• Weekend support may be required
• No on-call requirement
• Service area may extend up to approximately 60 miles or 60-90 minutes of total driving per day
• Uses Home Care Home Base (HCHB), Remote Client for Home Health, Rover phone application, and OASIS documentation
• Facility makes efforts to group patient visits to minimize driving
• Business casual attire required; no jeans; scrubs permitted but not required with no specific color
• Free parking available on-site
• Mileage reimbursed at the current IRS standard rate
• Up to 3 days of RTO may be automatically approved
• Blackout periods apply during the weeks of Thanksgiving, Christmas, and New Year's
• Local clinicians accepted within a 50-75 mile radius at the same pay rate
• Travel pairs accepted, with each traveler assigned to their own territory and required to have their own transportation
• First-time travelers are accepted for this position
