American Traveler is seeking an LPN for a skilled Home Health assignment in Oregon. Active Oregon license and at least 1 year of skilled Home Health experience are required. BLS certification and proficiency with HCHB and OASIS are also required. Travel provides mileage reimbursement and a dynamic patient-care setting.

The role emphasizes 1:1 patient care, with a typical day 8:00 AM to 4:30 PM, and potential weekend support. First-time travelers are not accepted.

• BLS certification required
• Minimum of 1 year of skilled Home Health experience required
• HCHB (Homecare Homebase) proficiency required
• OASIS experience required for staff completing Start of Care (SOC)
• Reliable personal transportation and active driver's license required
• First-time travelers not accepted

Responsabilités

• Skilled Home Health setting with a 1:1 patient ratio
• Approximately 30 points per week, up to 6 patients per day
• 8-hour day shift schedule (8:00 AM-4:30 PM), 5 days per week
• 4x10 and 3x12 schedules are not available
• Weekend support may be required
• Uses Home Care Home Base (HCHB), Remote Client for Home Health on laptop, and Rover phone application
• Facility makes efforts to group patient visits and minimize driving, though service area may extend up to approximately 60 miles or 60-90 minutes of total driving per day
• On-call rotation may be required; details discussed during interview
• Business casual attire required; no jeans; scrubs permitted but not required
• Free parking available; mileage reimbursed at current IRS standard rate
• Up to 3 days of RTO may be automatically approved; blackout periods include weeks of Thanksgiving, Christmas, and New Year's
• RTO requests must be submitted at time of application
• Travel pairs accepted, but each traveler assigned to own territory and must have own transportation
• Former permanent employees must be separated for a minimum of 6 months and approved by HR before returning as a traveler
• Local candidates accepted within a 50-75 mile radius at the same rate
• Monday orientation required, including completion of billable modules
