Job summary

American Traveler is seeking a Home Health LPN for a day-shift assignment in Southern Oregon. The role supports a 1:1 patient ratio, with about 30 productivity points weekly and up to six patients per day.

Applicants must hold an active Oregon LPN license, have at least 1 year of skilled home health experience, be HCHB proficient, and be able to work 8:00 AM–4:30 PM. Travel and mileage are reimbursed; RTO up to 3 days may be approved.

Qualifications

• Active Oregon LPN/LVN license required before start date; temporary licenses not accepted.
• Minimum 1 year of skilled Home Health experience required
• HCHB proficiency required
• OASIS experience required for SOC visits
• BLS certification required
• Must have reliable personal transportation and an active driver's license
• Full 7-year work history with gaps explained required for consideration
• Two professional references from a supervisor within the past 2 years required for consideration

Responsibilities

• Skilled Home Health setting with a 1:1 patient ratio
• Approximately 30 productivity points per week, up to 6 patients per day
• Day shift schedule, 8-hour shifts (8:00 AM-4:30 PM)
• Weekend support may be required
• No on-call requirement, though on-call rotation may be required per facility discussion
• Documentation completed using Home Care Home Base (HCHB), Remote Client laptop, and Rover phone application
• Visits are grouped by the facility to minimize driving, though service area may extend up to 60 miles or 60-90 minutes of total daily driving
• Business casual attire required; no jeans; scrubs optional with no specific color requirement
• Free parking available on site
• Mileage reimbursed at the current IRS standard rate
• Up to 3 days of RTO may be automatically approved; blackout periods apply during Thanksgiving, Christmas, and New Year's weeks
• All time off requests must be submitted at the time of application
• Monday orientation required, including completion of billable training modules
• Hospice/Home Health checklist completion is mandatory
• Travel pairs accepted, but each traveler is assigned their own territory and must have personal transportation
• Local candidates accepted within a 50-75 mile radius at the same rate
• Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
