American Traveler, LLC. is hiring a Home Health RN for skilled home health visits in Oregon. Position requires an active Oregon RN license, OASIS experience, and proficiency with HCHB, with 1+ year of home health experience.

Qualifications

• BLS certification required.
• Minimum of 1 year of skilled Home Health experience required.

• Valid driver's license and reliable personal transportation.

Responsibilities

• Skilled Home Health 1:1 patient care.
• Up to 6 patients per day with ~30 productivity points/week.
• Day shift, 8:00am–4:30pm; 4x10s/3x12s not available.
• Weekend support may be required.
• On-call rotation may be required; discuss during interview.
• Utilize HCHB, Remote Client for Home Health, Rover, and OASIS documentation.
• Group visits to minimize driving; travel up to ~60 miles/day.
• Up to 3 days of RTO may be automatically approved.
• Mileage reimbursed at IRS standard rate; free parking.

American Traveler is hiring a Home Health RN for a skilled home health setting requiring OASIS and HCHB experience and an active Oregon RN license.

Requirements

• Active Oregon RN license required before start date; temporary licenses not accepted
• BLS certification required
• Minimum of 1 year of skilled Home Health experience required
• OASIS experience required for completing Start of Care (SOC)
• Proficiency in Home Care Home Base (HCHB) required
• Must have a valid driver's license and reliable personal transportation

Responsibilities

• Skilled Home Health setting with a 1:1 patient ratio
• Up to 6 patients per day with approximately 30 productivity points per week
• Day shift schedule, 8-hour shifts from 8:00am to 4:30pm
• 4x10s and 3x12s schedules are not available
• Weekend support may be required
• On-call rotation may be required, to be discussed during interview
• Uses Home Care Home Base (HCHB), Remote Client for Home Health on laptop, Rover phone application, and OASIS documentation
• Facility makes efforts to group patient visits to minimize driving
• Service area may occasionally extend up to approximately 60 miles or 60-90 minutes of total driving per day
• Up to 3 days of RTO may be automatically approved
• Blackout periods include the weeks of Thanksgiving, Christmas, and New Year's
• Business casual attire required; no jeans; scrubs permitted but not required
• Free parking available; mileage reimbursed at current IRS standard rate
• Local candidates accepted within a 50-75 mile radius at the same rate
• First-time travelers are accepted
• Travel pairs accepted, though each traveler is assigned their own territory and must have their own transportation
• Former permanent employees must be separated for a minimum of 6 months before returning as a traveler
