Job summary

American Traveler is seeking an experienced LPN for a skilled Home Health position in Oregon. The ideal candidate has 1+ year of Home Health experience and an active Oregon license where applicable. Responsibilities include 1:1 patient visits, caseload management, and documentation using HCHB and Rover.

Day shift 8:00 AM–4:30 PM with possible on‑call and weekend support; mileage is reimbursed. Travelers welcome within a 60‑mile radius; business casual attire; benefits include health insurance

Qualifications

• Must hold an active Oregon LPN license (license required if applicable).
• Minimum 1 year of skilled Home Health experience.
• BLS certification required.
• OASIS experience required for SOC.
• Proficiency in HCHB required.
• Reliable transportation and valid driver's license.

Responsibilities

• Provide 1:1 skilled Home Health visits.
• Caseload of approximately 30 points per week (up to 6 patients/day).
• Day shift 8:00 AM–4:30 PM; other shifts not available.
• Weekend support may be required.
• On-call rotation may be discussed during interview.
• Documentation in HCHB and Rover; use of Laptop with Remote Client.

Job description

American Traveler is hiring an LPN for a skilled Home Health position requiring 1+ year of Home Health experience and active Oregon license (license required if applicable).

Requirements

• Active Oregon LPN license required before start date; temporary licenses not accepted (license required if applicable)
• Minimum 1 year of skilled Home Health experience required
• BLS certification required
• OASIS experience required for staff completing Start of Care (SOC)
• Proficiency in HCHB (Homecare Homebase) required
• Reliable personal transportation and valid driver's license required
• Skills required include medication administration, ear/eye drops, IM & SQ injections, NG/G tube care, oral and subcutaneous medication administration, inhaled medications, skin care, and wound care/wound vac

Responsibilities

• Skilled Home Health setting with a 1:1 patient ratio
• Caseload of approximately 30 points per week, up to 6 patients per day
• Day shift schedule, 5x8 hours (8:00 AM-4:30 PM)
• 4x10 and 3x12 schedules are not available
• Weekend support may be required
• On-call rotation may be required, to be discussed during interview
• Uses Home Care Home Base (HCHB) for documentation, Remote Client for Home Health on laptop, and Rover phone application
• Responsibilities include resumption of care and routine home health visits
• Facility groups patient visits to minimize driving; service area may extend up to approximately 60 miles or 60-90 minutes of driving per day
• Mileage reimbursed at current IRS standard rate
• Business casual attire required; no jeans; scrubs permitted but not required with no specific color
• Free parking available
• First-time travelers accepted
• Local clinicians accepted within a 50-75 mile radius at the same rate
• Up to 3 days of RTO may be automatically approved
• Blackout periods apply during the weeks of Thanksgiving, Christmas, and New Year's
• Former permanent employees must be separated for a minimum of 6 months and approved by HR before returning as a traveler
• Travel pairs accepted, with each traveler assigned to their own territory and required to have their own transportation
