Intermountain Health seeks a Clinical Program Coordinator in a full-time capacity to assess, plan, implement and coordinate trauma care workflows from diagnosis through follow-up. The role guides patients through the care team, provides educational support, and ensures adherence to evidence-based guidelines during hospital stays and after discharge.

The position requires a Bachelor's in Nursing, an active RN license in the state of practice, BLS-HCP certification, and at least three years of RN

Qualifications

• Bachelor's Degree in Nursing is required.
• Current license as a Registered Nurse in the state of practice.
• BLS-HCP certification required on hire or within 60 days of hire.
• Three years relevant RN experience.

Responsibilities

• Coordinates treatment processes from diagnosis through follow-up.
• Guides patients through collaborative care with all medical providers and support programs.
• Provides education to patients and families during hospital stay and post-discharge.
• Participates in patient care rounds.
• Oversees program compliance with regulatory requirements and performs data collection/analysis for performance improvement.
• Develops and implements evidence-based policies for the program.
• Maintains data for the program and serves as subject matter expert.
• Promotes mission, values, and standards; may float to other departments as needed.
