Job summary

Intermountain Healthcare Inc. is seeking a Clinical Program Coordinator at Good Samaritan Hospital in Lafayette, CO.

The role coordinates treatment processes from diagnosis through follow-up, guiding patients through collaboration with care providers and offering education throughout the stay and after discharge. The position involves rounds participation, policy development with the director, data collection and analysis for performance improvement, and maintaining a QA program.

Qualifications

• Bachelor's Degree in Nursing (minimum)
• Current RN license in the state of practice
• BLS-HCP certification on hire or within 60 days (ACLS/NRP/PALS as appropriate)
• Three years of relevant RN experience

Responsibilities

• Serves as coordinator for assigned program and discusses program with physicians and nurses to ensure care meets evidence-based guidelines; educates patients and families during hospital stay and after discharge.
• Participates in patient care rounds.
• Collaborates with the director to ensure program regulatory compliance; performs chart reviews and data collection; analyzes data for performance improvement.
• Collaborates with the director to develop and implement evidence-based policies and standards for the program; assesses plans and promotes continuing competence of site providers and students.
• Maintains and analyzes data for the program; serves as subject matter expert; creates and maintains a Quality Assurance program.
• Promotes mission, vision, and values of SCL Health; adheres to service behavior standards.
• Performs other duties; may float to other departments as needed; may be on-call during scheduled shifts.

Education

Bachelor's Degree in Nursing

Current RN license in state of practice

BLS-HCP certification

ACLS/NRP/PALS as appropriate
