Responsibilities

• Coordinate program activities and ensure care meets evidence‑based guidelines.
• Provide education to patients and families during the hospital stay.
• Participate in patient care rounds and data collection for performance improvement.
• Develop and implement evidence‑based policies for the program and promote provider competence.
• Maintain data and act as subject matter expert for the program; develop QA processes.
• May float to other departments and be on-call as needed.
