Intermountain Health is seeking a Clinical Program Coordinator to assess, plan, implement and coordinate trauma care processes from diagnosis through follow‑up, guiding patients through the collaborative process with physicians and care teams and providing education during the hospital stay.

Located at Good Samaritan Hospital in Lafayette, CO, this full‑time, on‑site role requires at least three years of RN experience, an active RN license, and BLS‑HCP certification; trauma experience is

Qualifications

Bachelor's degree in Nursing required.
Current RN license required in state of practice.
BLS-HCP certification at hire or within 60 days of hire.
Three years of RN experience required.

Responsibilities

Coordinate program: discuss with physicians/nurses and review care to meet guidelines; educate patients and family during hospital stay and after discharge.
Actively participate in patient care rounds.
Collaborate with director to ensure regulatory compliance and perform chart reviews; analyze data for performance improvement.
Develop and implement evidence-based policies and standards; promote professional development for care providers and students.
Maintain and analyze data for the program; serve as subject matter expert and establish a Quality Assurance program.
Promote mission, vision, and values of Intermountain Health; adhere to service behavior standards.
May float to other departments within scope, and may be on-call during scheduled shifts.
