Job summary

Intermountain Health seeks a Clinical Program Coordinator to assess, plan, and coordinate trauma patient care from diagnosis through follow-up. You will educate patients and families and collaborate with physicians, nurses, and support programs during the hospital stay and after discharge.

The role requires an RN with at least three years of experience, current BLS-HCP, and a willingness to work full-time on-site at Good Samaritan Hospital in Lafayette, CO. Master’s in Nursing is preferred.

Qualifications

• Registered nurse with current license in the practice state.
• Minimum three years of RN experience in a clinical setting.
• Evidence-based practice and quality improvement familiarity.

Responsibilities

• Coordinate treatment processes from diagnosis to follow-up.
• Guide patients and families through care with providers and support programs.
• Participate in care rounds and ensure regulatory compliance.
• Develop and implement policies for the program and professional development.
• Maintain data and quality assurance for the program.
• Promote mission and values; perform on-call duties as needed.
