Job summary

Intermountain Health in Lafayette, CO, seeks a Clinical Program Coordinator to assess, plan, implement and coordinate treatment processes from diagnosis through follow-up. You will guide patients through the collaborative care team and provide educational support during hospitalization.

This full-time role at Good Samaritan Hospital requires three years of RN experience, BLS‑HCP certification, and an active RN license in the state, with trauma-focused performance improvement experience preferred.

Qualifications

• Bachelor's degree in Nursing is required.
• Current license as a Registered Nurse in the state of practice.
• BLS‑HCP certification upon hire or within 60 days with approval.
• Three years of relevant RN experience.

Responsibilities

• Coordinate treatment processes from diagnosis through follow-up.
• Guide patients through the collaborative care process with physicians and nurses.
• Educate patients and family members during hospital stay and after discharge.
• Participate in patient care rounds and assist in performance improvement activities.
• Develop and implement evidence‑based policies and standards for the program.
