Job summary

Intermountain Health is seeking a Clinical Program Coordinator to assess, plan, implement and coordinate trauma care processes from diagnosis through follow‑up, guiding patients and families and collaborating with physicians and care teams during hospital stays.

This on-site role at Good Samaritan Hospital in Lafayette, CO requires RN licensure, a BSN, BLS-HCP, and at least three years of RN experience with a focus on performance improvement.

Qualifications

• Bachelor's Degree in Nursing and current RN license.
• Active BLS-HCP certification or obtained within 60 days.
• Three years of RN experience; trauma exposure preferred.

Responsibilities

• Coordinate treatment processes from diagnosis through follow-up.
• Educate patients and families during hospital stay and after discharge.
• Actively participate in patient care rounds.
• Review care for evidence-based guidelines and perform chart reviews for performance improvement.
• Collaborate to develop and implement policies; maintain continuing competence of care site providers.
