Responsibilities

• Coordinate assigned clinical program and ensure care aligns with evidence-based guidelines.
• Provide education to patients and families during hospital stay and after discharge.
• Participate in patient care rounds.
• Collaborate with the director to meet regulatory requirements and perform data reviews for improvement.
• Develop and implement evidence-based policies and promote professional development.
• Maintain data and serve as subject matter expert; create QA program.
• Support mission and values; may float to other departments as needed.
• May be on-call during scheduled shifts.
