Job summary

Health Connect America seeks a licensed behavioral health therapist to deliver individualized services to clients and families across our SC network. You will work within multidisciplinary teams to assess, plan, and implement evidence-based therapies, with a focus on trauma-informed care and client-centered outcomes.

The role requires a Master’s in Social Work or Counseling and an active LMSW/LMFT-A/LPC-A license, plus strong communication and collaboration skills.

Qualifications

• Master's Degree in Social Work, Counseling, or related field required.
• Must hold a LMSW, LMFT-A, or LPC-A license.
• Experience delivering behavioral health services with teams across settings is preferred.

Responsibilities

• Conduct biopsychosocial assessments and use evidence-based tools to identify client needs and treatment recommendations.
• Develop individualized treatment plans with measurable goals in collaboration with clients and families.
• Provide individual, family, and group therapy using evidence-based interventions for anxiety, depression, trauma, and behavioral concerns.
• Monitor client progress, communicate with care teams, and adjust interventions to improve outcomes.
• Provide crisis intervention, safety planning, and referrals to resources as needed.
• Maintain timely, accurate clinical documentation and ensure compliance with policies and regulations.
