Job summary

Health Connect America is hiring a Therapist to deliver individualized behavioral health services to youth and families. You will work with families, treatment teams, and community resources to support client success and implement evidenced-based interventions across individual, family, and group therapy formats.

The role emphasizes crisis intervention, documentation, and compliance with state regulations while collaborating with multidisciplinary teams in a client-centered care model.

Qualifications

• Master's Degree in Social Work, Counseling, or a related field required.
• Must hold a LMSW, LMFT-A, or LPC-A license.

Responsibilities

• Provide individualized behavioral health services with families and treatment teams.
• Develop and implement individualized treatment plans with measurable goals.
• Deliver individual, family, and group therapy using evidence-based interventions.
• Monitor progress, communicate with stakeholders, and adjust interventions as needed.
• Provide crisis intervention, safety planning, and referrals.
• Maintain timely clinical documentation and comply with policies and regulations.
