Job summary

Health Connect America seeks a licensed behavioral health Therapist to deliver high-quality services to clients in North and South Carolina. You will assess, plan, and implement evidence-based therapy, partnering with families and treatment teams to drive meaningful progress.

Ideal candidates hold a Master’s degree in Social Work, Counseling, or related field and an active LMSW/LMFT-A/LPC-A license, with experience in assessments, treatment planning, and crisis intervention.

Qualifications

• Master's Degree in Social Work, Counseling, or a related field required.
• Must hold a LMSW, LMFT-A, or LPC-A license.
• Experience conducting behavioral health assessments, treatment planning, and therapeutic interventions preferred.
• Strong knowledge of evidence-based treatment approaches, trauma-informed care, and crisis intervention practices.
• Excellent communication, documentation, collaboration, and relationship-building skills.
• Ability to work effectively within a multidisciplinary treatment team while managing an active caseload.
• Valid driver's license, reliable transportation, and current vehicle insurance required for community-based service delivery, when applicable.

Responsibilities

• Conduct comprehensive biopsychosocial assessments and utilize evidence-based assessment tools to identify client strengths, needs, and treatment recommendations
• Develop individualized treatment plans with measurable goals and objectives in collaboration with clients, families, and multidisciplinary treatment teams
• Provide individual, family, and group therapy using evidence-based interventions to address concerns such as anxiety, depression, trauma, behavioral challenges, and other mental health conditions
• Monitor client progress, maintain ongoing communication and follow-up, and adjust treatment interventions to support positive outcomes
• Provide crisis intervention, safety planning, and referrals to additional resources and support services as needed
• Maintain timely, accurate clinical documentation while ensuring compliance with organizational policies, payer requirements, accreditation standards, and state regulations
