Job summary

Health Connect America is seeking a licensed therapist to provide high-quality behavioral health services within the South Carolina service area. The role emphasizes individualized care, collaboration with families and treatment teams, and the use of evidence-based practices to support emotional wellness.

Responsibilities include assessment, treatment planning, therapy, crisis intervention, and care coordination, with a focus on trauma-informed care and client-centered outcomes.

Qualifications

• Master's Degree in Social Work, Counseling, or a related field required.
• Must hold a LMSW, LMFT-A, or LPC-A license.
• Valid driver's license, reliable transportation, and current vehicle insurance required for community-based service delivery, when applicable.

Responsibilities

• Conduct biopsychosocial assessments and use evidence-based assessment tools to identify client strengths, needs, and treatment recommendations.
• Develop individualized treatment plans with measurable goals in collaboration with clients and families.
• Provide individual, family, and group therapy using evidence-based interventions to address anxiety, depression, trauma, behavioral challenges, and other mental health conditions.
• Monitor client progress, maintain ongoing communication and follow-up, and adjust treatment interventions to support positive outcomes.
• Provide crisis intervention, safety planning, and referrals to additional resources and support services as needed.
• Maintain timely, accurate clinical documentation while ensuring compliance with policies, payer requirements, accreditation standards, and state regulations.

Education
