Transitions Mental Health Association in San Luis Obispo is seeking a Residential Case Manager to support adults in Community Housing and Bishop Streets Studios. You will supervise health, safety and social rehabilitation, promote independence, and work trauma-informed with residents.

The role offers a 40-hour week, Tue–Sat schedule, with Sunday/Monday off, and a comprehensive benefits package.

Qualificações

• BA/BS or AA/AS with target population experience.
• CADAC/CPRP certification accepted.
• Reliable automobile with mileage reimbursement.

Responsabilidades

• Provide case management for residents in Community Housing and Bishop Streets Studios.
• Assist with daily living skills, nutrition, budgeting, housecleaning and personal hygiene.
• Assess psychiatric symptoms; respond to crises with coping strategies.
• Promote trauma-informed practices and collaborative communication.
• Educate clients and families about mental illness and treatment options.
• Provide transportation to appointments and support community resources.
• Document services and complete program paperwork.
• Attend staff meetings and required trainings.
