Qualifications

• Associate's Degree preferred; or High School Diploma/GED with 2-5 years of equivalent experience in related field.
• Bilingual skills strongly preferred.
• Experience in homelessness services or outreach is a plus.

Responsibilities

• Develop and maintain individualized case plans focused on housing, employment, benefits, and overall well-being.
• Assist with housing searches, employment readiness, benefits enrollment (e.g., CalFresh), and budgeting skills.
• Maintain accurate client documentation and outcome tracking in HMIS and agency system.
• Coordinate outreach to encampments and connect clients to shelter, housing, and essential services.
• Provide crisis intervention and trauma-informed, empathetic case management to empower clients.
