Job summary

Santa Maria Hostel seeks an experienced Residential Case Manager to deliver trauma-informed, client-centered case management in an intensive residential program in Houston, TX. You will build rapport, coordinate access to services, and support recovery through referrals and follow-up.

The role emphasizes individualized service plans, collaboration with a multidisciplinary team, and culturally competent care, with a focus on family and community involvement as appropriate.

Qualifications

• Licensed or credentialed as an LCDC Intern (LCDCI) in Texas.
• Experience in substance use disorder or behavioral health settings preferred.
• Bilingual in English and Spanish strongly preferred.
• Valid Texas Driver’s License and proof of insurance if driving on Santa Maria business.

Responsibilities

• Establish rapport with clients using motivational strategies.
• Provide weekly case management sessions and care coordination.
• Develop and revise individualized service plans with the client and team.
• Coordinate communications with referral sources, healthcare providers, courts, CPS, and external systems.
• Facilitate referrals and linkage to mental/physical health and social services; assist with transportation and benefits as needed.
• Participate in treatment team meetings and staff reviews; adjust service plans as progress is made.
