Job summary

Transitions Mental Health Association is seeking an Outreach Worker for the FSP ICM program in San Luis Obispo. This field-based role focuses on engagement, care coordination, housing access, and linkage to community resources for adults with serious mental illness and co-occurring disorders.

You will work primarily in the community, building trust, coordinating care, and supporting recovery goals with trauma-informed, person-centered approaches.

• Commitment to helping adults with serious mental illness access services.

Responsibilities

• Identify homeless mentally ill adults and connect them with housing and services.
• Respond to crises and link clients to outpatient or crisis care.
• Help clients set goals, review progress, and pursue independence.
• Complete intake paperwork and track client progress.
• Coordinate with internal/external staff to meet program objectives.
• Attend weekly, monthly and all-agency meetings; share client information for feedback.
• Receive referrals and complete intake for potential clients; follow progress.
• Coordinate with nurses and mentors for holistic wellness support.
• Aim to outreache 300+ clients per year and connect them to treatment.
• Assist clients with obtaining IDs and applying for benefits and income supports.
• Support Medi-Cal billing and documentation to meet goals.
