Qualifications

• Trauma-informed case management experience with participants facing multiple barriers.
• Familiarity with housing stability programming and supportive services.
• Ability to document accurately in case notes and HMIS.
• Strong communication and collaboration with community partners.

Responsibilities

• Provide trauma-informed case management using motivational interviewing to participants.
• Engage with participants in various settings to promote housing stability.
• Document case notes, plans, leases, and required forms accurately.
