Job summary

YWCA South Hampton Roads is seeking a Case Manager to support shelter guests and community clients. You will conduct initial assessments, connect clients to services, and manage a caseload under supervision of the Director of Support Services.

You will educate clients on resources related to interpersonal violence, safety planning, and trauma-informed care while maintaining confidentiality and collaborating with Housing and Victim Services teams. Travel is required.

Qualifications

• Bachelor's degree or 8 years of related experience required.
• Experience working with victims of interpersonal violence, persons with disabilities, mental health concerns, and chronic homelessness.
• Strong communication and documentation skills; ability to work independently and in a team.

Responsibilities

• Conduct initial service intake and needs assessments to determine client needs and coordinate services based on intake assessment.
• Complete thorough assessments to identify strengths, needs, barriers, and goals.
• Provide ongoing case management, referrals, and crisis intervention for shelter and community clients.
• Maintain client case records and ensure confidentiality per policy.
• Coordinate with Housing, Victim Services and Allied Professionals to support clients.
