Chicago House is seeking a Housing and Health Case Manager for residential housing in Chicago. You will assess clients, develop service plans, coordinate with medical providers, and advocate for benefits to support health, income, and housing stability.

This exempt, full-time role requires trauma-informed care, motivational interviewing, and strong teamwork. On-call one week per month; bilingual abilities are a plus in serving diverse populations.

Qualifications

• Bachelor’s degree in Social Work or related field.
• Masters degree preferred for advanced practice.
• Experience with Motivational Interviewing applications.
• Trauma-informed care experience relevant to housing work.
• Familiarity with housing programs and social services.

Responsabilités

• Conduct psychosocial assessments and create service plans to meet client needs.
• Routinely assess progress and engage clients in motivational strategies.
• Provide case management to improve health outcomes, income, and housing stability.
• Advocate for benefits and entitlements for clients.
• Complete discharge planning and follow-up with clients.
• Attend staff meetings and participate in blended management.
• Report changes in housing or client health to supervisor.
• Maintain client files per Chicago House and funder standards.
