Job summary

Bayaud Works is seeking a Case Manager to provide trauma-informed, housing-focused case management for families at the Tamarac Family Emergency Shelter in Denver. You will identify housing barriers, develop stability plans, and connect residents with wraparound services to promote long-term stability and self-sufficiency.

The role requires collaboration with shelter staff, strong documentation, and adherence to Housing First standards.

Qualifications

• Associates degree in Social Work, Human Services, or related field preferred; equivalent experience considered.
• Minimum 2 years experience in case management, housing navigation, or human services.
• Experience with HMIS or similar data systems preferred.

Responsibilities

• Provide housing-focused case management, including intake, assessment, service planning, referrals, follow-up, and stabilization support.
• Develop and update individualized housing stability and employment plans with clear goals and steps.
• Support residents with housing navigation, documentation, landlord communication, benefits, and employment resources.
• Maintain accurate documentation in HMIS and program systems.
• Participate in case conferencing, team meetings, and coordinated service planning.
• Drive/transport clients to facilities and required events.
• Advocate with landlords, housing providers, and partners; support resident self-advocacy.
• Support crisis intervention, de-escalation, and harm reduction per procedures.
• Maintain caseload of ~30 households or fewer; achieve 75 touch points per month.
