Job summary

BrightSpring Health Services is seeking a Foster Care Case Manager to support trauma-informed, youth-centered services for children in foster care. You will coordinate case management, service planning, and collaboration with foster families, biological families, schools, and community providers to promote safety, stability, and positive outcomes.

Qualifications

• Bachelor's degree in social work, sociology, psychology, counseling, or related human services field is required.
• Two years' experience working with children and families required.
• A bachelor's degree in related fields with five years' experience at a placing agency may substitute for a graduate degree.

Responsibilities

• Reviews referrals and matches children with appropriate foster families.
• Places children in state licensed or state certified foster homes.
• Obtains required documentation for child in care.
• Provides weekly contact and bi-weekly visits to child and foster home.
• Prepares treatment plans for children within 21 days of placement and quarterly thereafter.
