Job summary

BrightSpring Health Services Corporation, through StepStone Family & Youth Services, is seeking a compassionate Foster Care Case Manager to coordinate and deliver individualized services for children and families. The role emphasizes trauma-informed care, safety, trust, healing, and resilience in collaboration with foster families and community providers.

The position focuses on advocating for youth, coordinating care, guiding families to resources, and ensuring culturally responsive services

Qualifications

• Bachelor's degree in a human services field or nursing field.
• Valid driver's license.
• Must have two years of experience with case management or related discipline.
• Must be able to communicate both verbally and in writing.

Responsibilities

• Receives referrals and contacts to identify client needs and consults with management.
• Completes pre-screening assessments and pre-admission paperwork including authorization and treatment team coordination.
• Oversees and supervises field staff supporting pre-screening processes.
• Accesses account information and contracts with payer organizations as appropriate.
• Consults with third-party representatives about client benefit coverage and reimbursement options.
• Contacts referral sources to advise case acceptance and provide information on clinical team responsible.
• Develops knowledge of services/resources provided by the Company and available community services.
• Maintains relationships with referral sources and payer case managers; tracks outcomes and referrals.
• Implements operational processes to ensure compliance with policies and regulatory mandates.
• Participates in HIPAA privacy practices and company ethics programs.
• Participates in growth planning and recruitment activities.
• Performs other duties as assigned.
