Job summary

StepStone Family & Youth Services, affiliated with BrightSpring Health Services, seeks a compassionate Case Manager focused on trauma-informed care for children and families. You will coordinate services, support families, and advocate for access to resources in a collaborative, community-centered setting.

Responsibilities include pre-screenings, referrals, and working with a multidisciplinary team, while ensuring culturally responsive care and safety for youth and families.

Qualifications

• Bachelor's degree in a human services field or nursing field.
• Valid driver's license.
• Two years of experience with case management or related discipline.
• Ability to communicate both verbally and in writing.

Responsibilities

• Receives and responds to incoming calls from referral sources and clients, identifies needs, and consults with the Director of Clinical Management.
• Completes pre-screening assessments, pre-screening reports, and case proposals; obtains authorization for payment.
• Oversees and supervises field staff assisting with pre-screening.
• Accesses account information, contracts, and payer agreements as appropriate.
• Consults with third party representatives on client benefit coverage and reimbursement procedures; negotiates with payers.
• Contacts referral sources to advise case acceptance and provide clinical team information.
• Maintains knowledge of all services provided by the company and community resources.
• Maintains relationships with referral sources and payer case managers; tracks referrals and conversions.
• Implements operational processes to ensure compliance with policies and regulatory mandates.
• Adheres to HIPAA privacy program and Business Ethics and Compliance programs.
• Participates in growth planning meetings and staffing discussions.
• Participates in special projects and other duties as assigned.
