Job summary

StepStone Family & Youth Services, an affiliate of BrightSpring Health Services, provides trauma-informed, youth-centered foster care services to children and families in care. The Foster Care Case Manager supports youth and families through case management, service coordination and individualized planning for safety, stability and permanency.

Qualifications

• The Foster Care Case Manager has a bachelor's degree in social work, sociology, psychology, counseling, or related human services field and two years' experience.
• A bachelor's degree in social work, sociology, psychology, counseling, or related human services field and five years' experience with a placing agency may be considered in lieu of a graduate degree.

Responsibilities

• Deliver case management services, individual and family counseling, and supervision of foster families.
• Develop individual treatment plans for troubled children and their families.
• Maintain weekly contact with foster families, children, and bio-families and monitor progress.
• Assist with training, home studies, and evaluation of foster parents.
• Use standardized tools to measure social and emotional progress and prepare plans and reports.
