Job summary

StepStone Family & Youth Services seeks a Foster Care Case Manager to support youth and foster families through trauma-informed, strengths-based practices and family-centered services.

The role focuses on developing individualized case plans, coordinating services, conducting regular visits, and maintaining accurate documentation to promote safety, stability, permanency, and well-being for youth.

Qualifications

• Bachelor's degree in a human services field or nursing field.
• Valid driver's license.
• Two years of experience with case management or related discipline.
• Strong verbal and written communication skills.

Responsibilities

• Receives and responds to referrals and identifies client needs; coordinates with the clinical management team.
• Conducts pre-screening and supports development of case proposals and admission paperwork.
• Oversees field staff assisting with pre-screening and ensures program readiness.
• Maintains knowledge of services and resources; identifies community alternatives when needed.
• Maintains HIPAA privacy and compliance with company policies; participates in growth planning.
