Qualifications

• Bachelor's degree in a human services or nursing field.
• Valid driver's license.
• Two years' experience in case management or related discipline.
• Strong written and verbal communication.

Responsibilities

• Receive/respond to referrals and assess client needs with the Director of Clinical Management.
• Complete pre-screening assessments and prepare case proposals and related paperwork.
• Oversee field staff assisting with the pre-screening process.
• Coordinate with third-party payers on benefit coverage and reimbursement procedures.
• Maintain relationships with referral sources and provide management with reports.
• Ensure compliance with HIPAA and company policies.
