Qualifications

• Bachelor's degree in a human services field or nursing field is required.
• Must have a valid driver's license.
• Must have two years of experience with case management or related discipline.
• Ability to communicate effectively both verbally and in writing.

Responsibilities

• Receives and responds to referrals to identify client needs and consults with the Director of Clinical Management to determine program readiness.
• Pre-screens referrals, completes documentation, and prepares case proposals and related admission paperwork.
• Oversees and supports field staff involved in the pre-screening process.
• Maintains knowledge of services and community resources; coordinates with foster families, schools, and service providers.
• Maintains HIPAA privacy and company compliance practices; participates in growth planning and special projects.
