Qualifications

• Bachelor's degree in a human services field or nursing.
• Valid driver's license.
• Two years of experience with case management or related discipline.
• Must be able to communicate both verbally and in writing.

Responsibilities

• Receives referrals and identifies clients' needs; consults with Director of Clinical Management to determine ability to meet them.
• Completes pre-screening assessments and related paperwork, including authorization for payment and coordinating the treatment team.
• Oversees and supervises field staff supporting the pre-screening process.
• Accesses client benefit coverage and reimbursement procedures; negotiates levels with payers as needed.
• Maintains knowledge of services offered and community resources; identifies alternative sources when needed.
• Maintains relationships with referral sources and payer case managers; tracks referrals and outcomes.
