Job summary

StepStone Family & Youth Services seeks a compassionate Foster Care Case Manager to support children and families through trauma-informed, strengths-based practice. You will develop individualized case plans, coordinate services, and document progress to promote safety, stability and permanency.

The role collaborates with foster parents, biological families, schools and community partners to ensure comprehensive, client-centered support and ongoing placement stability.

Qualifications

• Bachelor's degree in a human services field or nursing field.
• Valid driver's license.
• Must have two years of experience with case management or related discipline.
• Must be able to communicate both verbally and in writing.

Responsibilities

• Receives and responds to referrals and determines client needs in collaboration with management.
• Pre-screens, completes reports, and prepares case proposals and admissions paperwork.
• Supervises field staff assisting with pre-screening and case initiation.
• Maintains knowledge of services and community resources to coordinate care.
• Develops and maintains relationships with referral sources and payer representatives.
• Monitors referral source satisfaction and reports on conversion and outcomes.
• Ensures compliance with company policies and HIPAA/privacy practices.
