Qualifications

• Bachelor's degree in human services or nursing required.
• Valid driver's license required.
• Two years of case management experience preferred.
• Excellent verbal and written communication skills required.

Responsibilities

• Receives and responds to referrals, identifying client needs and consulting with Director of Clinical Management to assess capacity to serve.
• Completes viable referrals by pre-screening, preparing reports, and coordinating pre-admission paperwork (authorization for payment, treatment team availability).
• Oversees and supervises field staff involved in pre-screening processes.
• Accesses account information and past payer agreements as appropriate.
• Consults with third-party representatives on benefit coverage and reimbursement procedures; negotiates levels with payers.
• Contacts referral sources to inform case acceptance and assigns clinical team responsible for the case.
• Maintains knowledge of all services offered by the company and community resources; identifies alternatives when needed.
• Maintains relationships with referral sources and payer case managers; tracks outcomes and reports as needed.
• Implements operational processes to ensure policy compliance; adheres to HIPAA and ethics programs.
• Participates in growth planning and special projects as assigned.
