Qualifications

• Bachelor's degree in a human services field or nursing field.
• Valid driver's license.
• Two years of experience with case management or related discipline.
• Ability to communicate both verbally and in writing.

Responsibilities

• Receives referrals and identifies clients' needs with input from the Director of Clinical Management.
• Completes viable referrals by pre-screening assessments and related paperwork.
• Oversees field staff assisting with pre-screening processes.
• Accesses account information and contractor payer terms as appropriate.
• Consults on client benefit coverage and reimbursement procedures with third parties.
• Contacts referral sources to inform case acceptance and assigns the clinical team.
• Maintains knowledge of services and community resources; identifies alternatives when needed.
• Maintains relationships with referral sources and tracks satisfaction and conversions.
• Implements processes to ensure compliance with Company policies and regulatory mandates.
• Participates in HIPAA/privacy programs and annual compliance activities.
• Engages in growth planning and special projects as assigned.
