Job summary

Volunteers of America Illinois is hiring a Client Support & Service Coordinator for the SSVF Polk Street program in Chicago. You will provide client-centered case management, connect veterans to housing resources, and coordinate services with community partners to promote stability and well-being.

You will conduct assessments, develop service plans, and document progress while traveling to client sites and supporting a trauma-informed approach.

Qualifications

• 2+ years of experience in case management or related human services field.
• Experience with trauma-informed care and leveraging community resources.
• Strong communication, collaboration, and de-escalation skills.
• Proficiency in Microsoft Office and documentation systems.

Responsibilities

• Provide intensive case management and crisis intervention.
• Conduct client intake, assessments, and evaluate needs.
• Develop and monitor individualized service plans for client stability and progress.
• Conduct monthly home visits and community-based meetings.
• Advocate for clients and connect them with community resources and services.
• Coordinate services and referrals among clients, agencies, and providers.
• Educate clients on available services, applications, and rights.
• Maintain timely documentation in EchoVantage, HMIS, and required systems.
• Travel within the community and support housing stability goals.
