VOA Illinois is seeking a Client Support & Service Coordinator to provide client-centered case management and trauma-informed support for veterans and families. You will connect clients to community resources, monitor service plans, and promote housing stability and well-being throughout the program.

This on-site role offers hybrid eligibility after 90 days of initial training. The position requires a Bachelor’s degree in a related field and at least 2 years of experience in case management or

Qualifications

• Bachelor’s degree required in Human Services, Social Work, Psychology, or related field.
• At least 2+ years of case management or human services experience working with vulnerable populations.
• Experience with trauma-informed care and leveraging community resources.

Responsibilities

• Provide intensive case management and crisis intervention.
• Conduct client intake, assessments, and evaluate needs.
• Develop and monitor individualized service plans.
• Conduct monthly home visits and community meetings.
• Advocate for clients and connect them with resources and programs.
• Coordinate services between clients, agencies, and providers.
• Educate clients on available services, processes, and rights.
• Use trauma-informed care, active listening, and conflict resolution.
