Job summary

The Salvation Army Nashville Area Command is seeking a dedicated case management professional to provide comprehensive, long-term support to a caseload of clients within an established life management program. You will assess needs, design individualized plans, and work closely with clients and staff to monitor progress and outcomes.

Responsibilities include advocacy, resource coordination, and ensuring funding requirements are met while applying trauma-informed practices to improve stability

Qualifications

• Bachelor's degree from an accredited college or university in Social Work, Behavioral Science, or a related field.
• Three years of progressively responsible experience providing direct case management social services, including assessing clients’ needs and developing individual, comprehensive, long-term action plans for recovery utilizing a wide variety of resources.
• Or any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.

Responsibilities

• Interviews, accepts, and provides comprehensive, long-term, structured case management services for an assigned caseload of clients participating in an established life management program.
• Develops comprehensive program plans/goals and evaluates client's progress by conducting mentoring and counseling sessions with client and/or staff.
• Serves as advocate for client in order to acquire services that will enable them to functionally cope with their environment; ensures constant compliance with funding requirements.
• Case management requires assessing client needs, developing individualized service plans, establishing measurable goals, coordinating services, maintaining records and HMIS documentation.
