Job summary

The Salvation Army Tucson is seeking a Case Manager to provide professional, individualized case management and referrals to help homeless participants address underlying barriers and move toward independence.

Responsibilities include weekly meetings, psychosocial assessments, ISP development, job readiness coaching, budgeting, HMIS data entry, collaboration with housing resources, and reporting, with availability to evenings, weekends, and holidays.

Qualifications

• BA/BS degree in social work or related field required.
• TB clearance prior to hire.
• CPR and First Aid certification prior to employment.
• Knowledge of homeless populations and available resources.

Responsibilities

• Meet with participants weekly to assess progress and provide support.
• Develop an ISP and monthly action plan to achieve housing goals.
• Coach in job search, resume writing, interviewing and follow-up.
• Maintain HMIS/Clarity records with case notes within 72 hours.
• Collaborate with housing resources and community partners.
