Job summary

Lutheran Services Florida, Inc. is seeking a Case Manager Intensive to provide trauma-informed, intensive case management to vulnerable clients.

The role includes conducting weekly in-person trauma support, developing self-sufficiency plans, and coordinating referrals within a culturally appropriate framework. Requirements include a Bachelor’s degree (social work or related field) and prior case management experience.

Qualifications

• Bachelor’s degree required. Preference for degree in Social Work or behavioral/human services field.
• Two years of experience providing case management and/or social services preferred.
• Case management experience strongly preferred.
• 3 years experience working with refugees or immigrants preferred.
• Cross-cultural or international experience preferred.

Responsibilities

• Provide trauma-informed intensive case management and supportive services.
• Conduct trauma-focused in-person weekly sessions for crisis intervention and safety planning.
• Develop goal-oriented, client-centered self-sufficiency plans.
• Maintain timely communication with supervisor regarding client safety and well-being.
• Submit thorough documentation per LSF policies and procedures.
• Coordinate referrals and ensure services are linguistically and culturally appropriate.
• Provide training and supervision to interns and volunteers.
• Maintain comprehensive, confidential case files.
