YFA (Youth and Family Advocates) is seeking an Intensive Services Case Manager to provide comprehensive case management for youth and families in Pasco, FL. The role emphasizes trauma-informed care and collaborative work with a multidisciplinary team to ensure permanency and safe family environments.

Responsibilities include facilitating assessments, planning, linking to resources, and advocating from intake through discharge, with a focus on reducing placement instability and improving outcomes

Qualifications

• Bachelor’s degree and CWCM certification are required.
• Must demonstrate ability to travel regionally and statewide.
• Experience in case management and youth services is preferred.

Responsibilities

• Provide case management services to assigned children and families from intake to discharge.
• Deliver intensive case management with safety planning, crisis intervention, and multidisciplinary coordination.
• Perform Family Finding to connect youth with relatives, supports, and mentors.
• Develop and monitor service and case plans with measurable permanency goals within 12 months.
• Maintain accurate electronic and paper records and ensure timely documentation.
