Job summary

Lutheran Services Florida, Inc. is seeking a Child Welfare Case Manager to provide wrap-around case management services and build strong relationships with children and families served.

The role emphasizes timely home visits, detailed FSFN documentation, and collaboration with guardians, foster parents, and service providers to pursue reunification or other permanency outcomes. Ideal candidates bring a Bachelor’s degree in a Human Services field and at least one year of relevant experience.

Qualifications

• Bachelor’s degree in a Human Services field is required.
• Minimum of one year of relevant experience with children.
• Must obtain child welfare certification within one year of hire.

Responsibilities

• Meet with children in their primary residence within 2 working days of staffing.
• Develop case plans with families, guardians, and other stakeholders aiming for reunification or permanency.
• Conduct required home visits and document activities in FSFN within 48 hours.
• Submit service requests and refer clients to community resources; maintain knowledge of resources and eligibility.
• Attend court activities and participate in staffing and packets as needed.
• Coordinate emergency placements and services for at-risk children.
• Arrange and facilitate parental and sibling visits; transport children as needed.
• Ensure Independent Living functions are completed and deadlines met.
